This lecture provides a comprehensive clinical guide to immediate implant placement following tooth extraction. It focuses on the biological and prosthetic principles required to achieve predictable outcomes while preserving hard and soft tissue architecture.
Through clinical cases, the lecturer explains implant selection, primary stability, three-dimensional positioning, socket classification, bone grafting, provisional restoration design, and soft tissue management in both posterior and anterior esthetic regions.

The lecture begins with the historical background, advantages, and clinical limitations of immediate implant placement. It examines common complications associated with earlier techniques, including buccal bone resorption and prosthetically unfavorable implant positioning.
The key factors for successful immediate implant placement include implant design, primary stability, prosthetically driven three-dimensional positioning, and preservation of hard and soft tissues. The lecture also explains the use of bone graft materials and hyaluronic acid to maintain the buccal contour.
Methods for preserving the soft tissue architecture and emergence profile using provisional restorations and customized healing abutments are presented. The contour of the provisional restoration is explained according to the C, B, and E Zones.
For posterior cases, extraction sockets are classified as Type A, B, or C based on the morphology of the interradicular bone. Appropriate drilling protocols, implant positioning, and implant diameter and length selection are discussed for each socket type.
The lecture concludes with immediate implant placement in the anterior esthetic zone, covering palatal implant positioning, connective tissue grafting, provisional restoration fabrication, and the management of clinical complications.
- Understand the advantages and clinical limitations of immediate implant placement after tooth extraction.
- Explain the importance of implant design and primary stability in immediate implant placement.
- Establish prosthetically driven implant positioning in the mesiodistal, buccolingual, and apicocoronal dimensions.
- Understand how bone graft materials and hyaluronic acid can be used to preserve the buccal contour.
- Manage soft tissue architecture and the emergence profile using provisional restorations and customized healing abutments.
- Design provisional restoration contours according to the C, B, and E Zones.
- Classify posterior extraction sockets as Type A, B, or C and select an appropriate implant placement strategy.
- Apply proper implant positioning and soft tissue augmentation techniques in immediate implant placement within the esthetic zone.
