Oral Surgery

Posterior Region

This lecture provides a comprehensive clinical guide to immediate implant placement in posterior regions, addressing the unique anatomical challenges of molar and premolar extraction sites. Starting with patient selection and indications, the presentation systematically covers the Smith & Tarnow classification of molar extraction sockets (Type A, B, and C) and the corresponding surgical strategies for each scenario—including septal bone engagement, gap management with or without bone grafting, and sinus floor elevation via osteodensification. Through detailed clinical case demonstrations, attendees will learn how to achieve predictable primary stability, manage primary wound closure, and optimize long-term restorative outcomes in posterior immediate implant cases.

Course Description Course Overview & Learning Objectives
Course Overview

This lecture presents clinical strategies for immediate implant placement in the posterior region based on different extraction socket conditions. It covers key considerations such as achieving primary stability, managing the jumping gap, implant placement depth, and emergence profile. Through clinical cases, Dr. Zhouyu Ding also demonstrates a minimally invasive approach using sticky bone and customized healing abutments to support stable healing and maintain a natural soft-tissue contour.

Learning Objectives

 - Understand the indications and patient-selection criteria for immediate implant placement in posterior regions

 - Master the Smith & Tarnow classification of molar extraction sockets (Type A, B, C) and apply site-specific surgical strategies

 - Learn techniques to achieve predictable primary stability, including septal bone engagement and controlled apical extension

 - Gain decision-making criteria for gap management: when spontaneous bone formation is predictable (<2 mm) versus when bone grafting is indicated (>2 mm)

 - Identify posterior anatomical limitations—inferior alveolar nerve and maxillary sinus—and strategies to avoid surgical complications
