Long-term implant success depends not only on osseointegration, but also on the establishment and maintenance of stable peri-implant soft tissues. This lecture introduces the concept of Surgical Soft Tissue Roadmaps, providing a systematic approach to evaluating peri-implant soft tissue conditions and selecting appropriate surgical soft tissue management strategies for different clinical situations. The lecture classifies clinical conditions according to factors such as soft tissue phenotype, keratinized tissue, and labial contour, and explains different treatment approaches for both healed sites and immediate implant sites through clinical cases. Various soft tissue augmentation procedures, including the roll flap, modified roll flap, and connective tissue graft (CTG), are also discussed to demonstrate how functional and esthetic peri-implant tissue can be established. Ultimately, the lecture presents a clinical decision-making framework aimed at achieving long-term functional stability and optimal pink esthetics with minimal and appropriate surgical intervention.
